Comprehensive Rehabilitation Program

Western Maryland Hospital Center’s Rehabilitation Program is a program of coordinated and integrated medical and rehabilitation resources. The program requires your active participation in planning your care and achieving your goals. A team of professionals will work with you to develop the plan and carry it out. One of the major goals of the unit is to discharge you to the community of choice.

The rehab team is concerned about your ability to be as independent as possible upon discharge. We will teach you new skills and relearn some old ones. We encourage you to ask questions and express any concerns. We are here to help you prepare to go home or a more independent level of care. Your treatment team may recommend a trial stay in the transitional living apartment prior to discharge in preparation to go home.

A patient in the Rehab has their plan of care reviewed in care planning conferences held twice a month. Family members are encouraged to attend. To find out more information about your care plan conference talk with your case manager.

Program Location

From the main lobby area, you should proceed to the ground floor elevator. In the elevator, select "2" to go to the second floor. Turn left when exiting the elevator, then turn left again at hallway. Walk toward the end of the hallway. At the end, the area to the right is identified as 2 East. This is where the rehabilitation program is located.
